<li>Empower patients and families to reach healthcare goals by educating them on disease management</li>
<li>Case manage and provide clinical oversight to interdisciplinary team</li>

<p><b>Home Health Registered Nurse (RN) Requirements</b></p>
<ul>
<li>RN license in the state you work</li>
<li>Two years of experience as a registered nurse, home health experience preferred</li>
<li>Current driver’s license</li>
<li>Current CPR and BCLS certification</li>
