This role focuses on evaluation, treatment planning, functional rehabilitation, and collaboration with interdisciplinary care teams to help patients achieve maximum independence and quality of life. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates PHYSICAL THERAPIST - HOME HEALTH | RESPONSIBILITIES: Perform physical therapy evaluations and treatments based on physician orders and referrals Conduct comprehensive patient assessments using appropriate evaluation tools Review patient medical history, diagnoses, precautions, and contraindications Develop individualized treatment plans with measurable goals Reassess patient progress and modify treatment plans as needed Communicate patient needs and progress with physicians, care teams, patients, and families Recommend and assist with adaptive equipment to improve patient function Maintain accurate evaluation, treatment, and billing documentation in compliance with regulatory requirements Adhere to professional standards, compliance policies, and HIPAA regulations Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice acts Participate in staff development activities and maintain required continuing education PHYSICAL THERAPIST - HOME HEALTH | REQUIREMENTS: Education: High School Diploma or GED (verifiable) required Attendance: Commitment to mandatory attendance during the first 90 days.
