About the Role
Job Description: Plans, coordinates, and implements Social Services for patients of all ages, patients’ families, and other person significant to patients at the Medical Center. Anticipates and attends to patient’s specific psychosocial needs, financial needs/concerns, equipment needs, educational needs, and community resources from admission through transfer to the next appropriate level of care. Assists with discharge planning, grief counsel, financial application, and access to community resources as needed. Enhances patient care, thus contributing to the goals and mission of the Medical Center. Education Qualifications: Educational Requirements: A Master's degree in social work from an accredited social work (MSW) program Licensure/Certification Qualifications: Certification/Licensure Requirements: Current State Social Work Licensure Duluth Hospice: Possession of a valid motor vehicle operator’s license as verified through appropriate licensing agency. Proof of current motor vehicle insurance.
