The Role To provide a safe and effective patient focused specialist clinical pharmacy service to a specified patient group(s) supporting medicines governance and quality improvement activities within specialist area(s) to ensure safe, effective and patient-centred systems in place. In this key role, you will: To deliver patient-centred specialist pharmaceutical care to patients in specified clinical area(s). To communicate effectively, placing the person at the centre of any interaction. To deliver patient education to ensure the safe and effective use of medications and devices. To collaborate with the wider pharmacy and multidisciplinary team; actively participating in consultant ward rounds and multidisciplinary team meetings. To deliver specialist clinical care to patients by assessment of their pharmaceutical care needs and developing and actioning a care plan. To carry out consultations with patients in pharmacist-led clinics and in ward, prescribing where appropriate. To review and analyse drug charts/prescriptions and supervise and participate in the dispensing and clinical and technical checking of discharge prescriptions and aseptically prepared products including systemic anticancer therapies (SACT). To review and analyse complex medicine and patient information and provide specialist advice on drug administration and changes to therapy accounting for individual patient needs and clinical condition and local management protocols. Prescribing where appropriate. What You'll Bring: Masters of Pharmacy Degree Registered member of General Pharmaceutical Council Independent prescribing qualification or must work towards Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma Relevant post registration experience in clinical pharmacy. Ability to effectively monitor patients’ therapeutic regimens in order to ensure effectiveness and reduce toxicity Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
