Transitions Social Worker - Learning Disabilities Location: High Street, Huddersfield, Huddersfield, West Yorkshire, HD1 2LE Working Pattern: Hybrid Eden Brown Synergy is currently recruiting for an experienced Transitions Social Worker to join a team in Kirklees. We are seeking a qualified and experienced Social Worker with a strong background in Learning Disabilities (LD) and transitions social work. The successful candidate will support young people transitioning from children's services into adult social care, ensuring they receive appropriate support and services to promote independence and wellbeing.
