Case Managers Compensation The pay for this position is 27.70 per hour Case Managers Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs The required availability for this position is 8am to 5pm CST time zone Hybrid position, involves field work and WFH telephonic. Mileage is reimbursable. Case Managers Responsibilities Facilitate healthcare outcomes by assisting with appointment scheduling and accessing benefits. Manage a telephonic caseload ranging from 250 to 500 members, depending on needs. Collaborate with members, providers, and community organizations to address health care and social needs. Use corporate software applications such as MS Word, Excel, and Outlook effectively. Case Managers Requirements Case management and discharge planning experience 2 years’ experience in behavioral health, social services or appropriate related field equivalent to program focus Managed Care experience Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually Bachelor's degree in behavioral health or human services is required.
