RN Case Manager - Part-time Weekends, Day Shift Hours (Temple Health-Chestnut Hill Hospital)

Part Time: The Case Manager provides clinically based case management to support the delivery of effective and efficient patient care in the inpatient or outpatient rehabilitation setting. Coordinates the rehabilitation team to establish appropriate patient-centered discharge plans in a timely fashion. Assists the patient and/or family in adjusting to and coping with lifestyle changes precipitated by episodes of illness or disability and in carrying out discharge recommendations. Uses a variety of modalities to assist patients and families in the alleviation or resolution of social, financial, and emotional problems related to illness, health care, and rehabilitation.<br/><br/>Education<br/>Other Graduate of an accredited school of nursing Required<br/><br/>Experience<br/>General Experience Relevant Experience Required<br/><br/>Licenses<br/>PA Registered Nurse License Required
