RN FIELD CASE MANAGER | DETAILS AND COMPENSATION:
- Location: Beckley WV 25801 – 100% on-site, home health settings
- Coverage Area : Raleigh, Wyoming, and Logan counties
- Payrate: $56.14/hr + mileage reimbursement
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM
- Productivity Requirement: 30 hrs per week
RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Field Case Manager will perform comprehensive assessments of high-risk members and coordinate individualized care plans. In this role, you will collaborate with primary care providers, social workers, and discharge planners to ensure appropriate levels of care and facilitate smooth transitions for patients. You will also play a key role in identifying non-medical support—such as housing and transportation—to ensure treatment plan compliance and optimal clinical outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

RN FIELD CASE MANAGER | RESPONSIBILITIES:
- Comprehensive Assessment: Evaluate high-risk members and continuously reassess for cost-efficient, appropriate levels of care.
- Care Coordination: Collaborate with PCPs to implement comprehensive care plans and coordinate transitions between healthcare settings.
- Member Advocacy: Act as a patient advocate while maintaining strict privacy and confidentiality; identify opportunities for health promotion and illness prevention.
- Administrative Oversight: Maintain detailed clinical, functional, and fiscal records throughout the management process.
- Resource Management: Engage specialty resources and establish non-medical supports to minimize adverse patient outcomes.
RN FIELD CASE MANAGER | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing or a related field.
- Attendance: Commitment to mandatory attendance during the first 90 days.
- License: Current, unrestricted Registered Nurse (RN) license in the state of West Virginia.
- Case Management certification is required.
- Proficiency in HCHB (Homecare Homebase) is required.
- Strong skills in analyzing complex medical information to make rational, evidence-based decisions.
- Ability to cover Raleigh, Wyoming, and Logan counties (mileage reimbursement provided).
