About the Role

We are looking for an experienced Qualified Social Worker to join Adult Services in Flintshire, supporting older people and their families. This is a Level III role suited to a confident practitioner who can manage complex cases independently, assess and manage risk, and make sound professional judgements in high-pressure situations. Location: Tŷ Draig, St David's Park, Deeside, Flintshire, CH5 3FF, United Kingdom Rate: £255.52 per day (Limited/Umbrella) Key Responsibilities: Undertake community care and safeguarding assessments in line with statutory requirements Identify risks, needs, and appropriate care solutions Develop and implement outcome-focused care plans Manage a complex caseload, including high-risk and safeguarding cases Commission cost-effective care packages to improve service users' quality of life Work closely with individuals, families, carers, and communities to support informed decision-making Monitor, review, and evaluate care plans to ensure effective outcomes Liaise with internal teams, health professionals, and external agencies Maintain accurate case records and produce reports as required Provide advocacy and, where necessary, give evidence in court Requirements: Qualified Social Worker (CQSW, CSS, DipSW or equivalent) Minimum 2 years' post-qualification experience Experience working with older people and their families/carers Strong assessment, risk management, and care planning skills Ability to work independently and manage complex caseloads Current Social Care Wales registration (or willingness to register) Key Skills & Attributes: Ability to work in a multi-disciplinary environment Strong communication and decision-making skills Commitment to promoting independence and person-centred care Ability to build effective relationships with service users and professionals Adaptability and resilience in a fast-paced environment How to Apply: If you are interested, please send your up-to-date CV to Shristi Yadav at or call 0114 700 3826 for more information.
