As a Senior Practitioner, your key responsibilities will include: Conducting comprehensive assessments to identify the needs and aspirations of young adults. Collaborating with families, educational institutions, and other stakeholders to develop tailored transition plans. Providing expert advice and support to junior staff, fostering a culture of learning and development. Monitoring and reviewing care plans to ensure they remain relevant and effective. Promoting independence and life skills among young adults through targeted interventions. To be considered for this role, you must possess the following qualifications and skills: A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current Social Work England registration. Significant experience in social work, particularly in supporting young adults with disabilities. Strong knowledge of legislation and best practices related to preparing for adulthood. Excellent communication, interpersonal, and organizational skills.
