Job Description
  • Provide care across 13 clinics in Chicago, Evanston, Brookfield, and Oak Park.
  • Work collaboratively with a dedicated clinical care team to deliver evidenced-based care.
  • Perform assessments, diagnoses, treatment, management of health problems, and health counseling.
  • Requirements include prior primary care or urgent care experience.
  • Submit BLS and license documentation; BLS from ARC or AHA required.

Critical Information
  • Average 30-35 hours per week; Monday - Friday from 8:30 AM to 7:30 PM, alternating weekends.
