We are seeking a highly skilled and compassionate nurse to join our growing Advanced Heart Failure team. In this role, you will support a complex and expanding patient population while working closely with cardiologists, advanced practice providers, and multiple medical departments to ensure that each patient receives seamless, well‑coordinated care. The position requires a nurse who is confident in implementing evidence‑based nursing interventions tailored to the unique needs of Advanced Heart Failure patients. While prior experience in cardiology or in managing medically complex patients is preferred, we also welcome candidates who demonstrate a strong desire to learn the intricacies of cardiovascular care, including, but not limited to, guideline-directed medical therapy (GDMT), advanced and supportive therapies (diuretics, electrolyte management, inotrope basics, anticoagulation management, and medication titration principles), and ability to do focused HF assessments.
Daily responsibilities include communicating with patients through telephone outreach, MyChart messaging, and in‑person interactions during clinic visits. Additionally, this role involves initiating and organizing patient education specific to cardiology and ensuring that individuals and their families have the information and resources necessary to navigate their care. Continuous learning is an important aspect of this position, and the nurse in this role will be encouraged to participate in ongoing professional development and quality improvement efforts within the service.
The Nurse Clinician will work in a multidisciplinary environment which will require exceptional interpersonal skills, including the ability to establish and maintain effective relationships with patients, physicians, clinical and administrative units. This position will utilize nursing skills to perform patient assessments via phone or in person and implement nursing interventions; as such the ability to use appropriate judgment, independent thinking and creativity when resolving patient care issues. Also responsible for initiating & organizing patient education for the population this unit serves.
