Qualified Social Worker - First Response TeamLocation: 1 Time Square, Warrington, Cheshire, WA1 2NT, United Kingdom Working Pattern: Hybrid Rate: £30 per hour Contract: 3 Months (Initial)
An exciting opportunity has arisen for an enthusiastic and motivated Qualified Social Worker to join the First Response Team within Adult Social Care in Warrington. This role is ideal for a practitioner who is passionate about delivering timely and effective support to adults with a wide range of needs.
The First Response Team works within a prevent, reduce, delay model, ensuring individuals are connected to the right services at the right time to promote independence and enhance wellbeing.
About the TeamThe First Response Team is the second point of contact for all new referrals into Adult Social Care. The team is responsible for:
Screening and triaging referrals received by Adult Social Care
Managing and reporting Safeguarding Adult Concerns
Supporting the Local Authority's duty to provide the AMHP service to coordinate assessments under the Mental Health Act (1983 as amended 2007)
The team ensures a seamless journey through adult social care, providing early intervention and short-term case management where required.
Key ResponsibilitiesAs a Social Worker within the First Response Team, you will:
Screen and triage new social care referrals through duty
Manage short-term caseloads
Undertake Care Act 2014 assessments to determine eligibility
Complete Carers Assessments
Carry out Mental Capacity Assessments and Best Interest Decisions
Conduct Safeguarding screenings and Section 42 enquiries
Contribute to Continuing Health Care (CHC) processes
Review and audit the delivery of care packages
Apply a strengths-based approach to practice
Work collaboratively with individuals, families, carers and partner agencies to achieve holistic outcomes
RequirementsTo be considered for this role, you must have:
Qualified Social Worker status
Registration with Social Work England
Experience working with adults with complex needs
Strong knowledge of safeguarding and risk assessment
Practical experience working within key legislation including:
Care Act 2014
Mental Capacity Act 2005
Mental Health Act 1983 (amended 2007)
