- Coverage Area:Escambia, Santa Rosa, Okaloosa, Walton Counties (expected to work in 2 counties: Escambia/Santa Rosa or Okaloosa/Walton)
Pay: $37.30/hr.
- Mileage Reimbursement:$0.49/mile
Schedule: Monday to Friday, 8:00am-5:00pm, 40 hours/wk. On-Call: 1 day per week
Overview
We’re hiring an LPN to provide in-home nursing care while maintaining each client’s dignity, safety, and independence. This is an onsite, field-based home health role supporting patients across assigned counties in the Pensacola area.
Key Responsibilities
- Provide skilled nursing care per the client’s plan of care and physician orders
- Monitor vital signs and report abnormal findings promptly
- Administer medications and treatments as ordered (including oxygen support, feedings, ostomy care when applicable)
- Perform wound care, assessments, and routine skilled nursing visits
- Coordinate and follow care routines established by PT, OT, and Speech Therapy
- Maintain infection control practices and report potential infections/communicable diseases
- Document all care provided, patient progress, and outcomes accurately and timely
- Order supplies/medications as needed to ensure appropriate stock on hand
- Monitor incidents, assess the patient after incidents, and complete required incident reporting
