About the Role
Social Worker required for work within the Adult Learning Disabilities Team in the beautiful county of Wiltshire, based Trowbridge. Competitive rates of pay rate from £36ph. 37 hours per week. 3 month minimum contract with options to extend. Flexible, hybrid working arrangements.
An exciting opportunity has arisen for an experienced social worker to join the Adult Learning Disability Team in Trowbridge. We work with people aged 18 and over who have a diagnosed learning disability to lead independent, fulfilled lives. We also work with families, carers, care providers and professionals to ensure that people with learning disabilities are able to access the knowledge, information and support they require to lead their lives as independently as possible.
The successful candidate will manage a caseload of work, using person centred practice principles to assess, support, plan and review. You will be required to complete assessments and develop personalised care packages for adults with learning disabilities. Duties include assessments, reviews, personalised budgets, person centred care planning, provision of care packages, liaising with other services, safeguarding adults. The successful candidate will thrive on creative thinking and problem solving and enjoy working autonomously. You will possess excellent communication and risk management skills.

You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
