JOB ADVERT - Social Worker (Hospital Discharge Team) - Slough Council Rate: £36.81 per hour Location: Wexham Park Hospital / Observatory House Contract: Locum / Agency Slough Council is currently recruiting for an experienced Social Worker to join their Hospital Discharge Team. This is a fast-paced role requiring confident practitioners who can deliver safe discharges within strict timescales using the D2A (Discharge to Assess) model. Key Requirements Ideally 5+ years post-qualified experience Strong knowledge of hospital discharge processes and D2A Able to complete assessments and progress discharge within 3-6 days Confident working under pressure and making quick decisions Experience using LAS is desirable Safeguarding experience essential Working Arrangements Main hospital base: Wexham Park Hospital Travel required for assessments and meetings 1 day duty based at the hospital 1 day based at Observatory House Hospital parking permit can be arranged This is a supportive and friendly team offering an excellent opportunity for experienced hospital-based social workers.
