Qualified Social Worker - Community Learning Disability Team - Bolton
£30 per hour | Hybrid Working

We are offering an exciting opportunity to join a focused and dynamic Community Learning Disability Team in Bolton, supporting the development of a wide range of services to meet the current and future needs of local people.

This is a fantastic role within a large multidisciplinary team, providing exposure to complex casework, formal decision-making forums, and court processes.

We are seeking an enthusiastic and motivated Social Worker with a genuine passion for supporting adults with learning disabilities-understanding their needs, aspirations, and helping to enhance their quality of life.

Key Responsibilities:

• Carry out Care Act assessments and develop effective, asset-based support plans
• Complete carers assessments
• Undertake mental capacity assessments and Best Interest decision-making
• Conduct Continuing Healthcare (CHC) assessments
• Participate in multi-agency and interdisciplinary working
• Manage safeguarding investigations
• Work within Court of Protection / DoLS frameworks

Requirements:

• Qualified Social Worker with relevant registration
• Experience within Learning Disability services
• Strong assessment and care planning skills
• Ability to work effectively within a multidisciplinary team
• Experience using Liquid Logic
• Ability to complete timely assessments and documentation
• Car driver essential

Working Arrangements:

• Initial 4 weeks office-based for induction
• Flexible hybrid working thereafter
