<p>Title: Physical Therapy Assistant<br />
Location: Norwalk, CT<br />
Salary: $60,000 to $70,000, DOE </p>
<p>Responsibilities:<br />
– Works closely with licensed Physical Therapist in home care setting<br />
– Documents notes in EMR system<br />
– Performs routine physical therapy tasks<br />
– Involved in performance improvement<br />
– Adheres to all compliance and regulatory standards of organization<br />
– Works closely with home health aides to deliver care<br />
– Speaks with patient’s family about education and training </p>
<p>Requirements:<br />
– Associate’s Degree in Physical Therapy<br />
– Preferred experience in home care setting<br />
– Experience with EMR systems<br />
– Valid Connecticut driver’s license<br />
– Must work well with staff members and patients</p>
