As the Hospice RN Admissions Nurse, you would be working alongside a supportive team and have a multitude of job functions. Please see job highlights below for more information.</p> <p>Title: Hospice Admissions RN<br /> Location: Manhattan NY<br /> Salary: $90,000 to $120,000</p> <p>Responsibilities:<br /> Completing assessments of patients as assigned<br /> Working closely with care coordinators<br /> Completing safety assessments of patients<br /> Documenting necessary notes in EMR system<br /> Reviewing medications and providing guidance to patients<br /> Communicating with patient family members when needed<br /> Working with Hospice staff and completing clinical reports<br /> Performing in depth assessments of patient’s physical health<br /> Obtaining physician orders during time of admission<br /> Providing documentation for all equipment and supply orders needed for patient<br /> Educating family members on patient care<br /> Reviewing medical history and providing ongoing active treatment </p> <p>Requirements:<br /> Associates Degree of Nursing<br /> At least 1 year of experience in long term care setting, hospital, hospice, or oncology<br /> Prior Admissions nursing experience a plus<br /> Active New York state RN license<br /> BLS<br /> Strong interpersonal and communication skills</p> <p>For further consideration, send resume or email to Alexg@smitharnold.com VP of recruitment or call 914-774-8202</p>
