Coordinate discharge planning with case management.</li><li style="font-size: 9pt; font-family: Verdana,sans-serif; color: #000000; font-weight: 400;" aria-level="1">Recommend, participate in, and work to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.</li><li style="font-size: 9pt; font-family: Verdana,sans-serif; color: #000000; font-weight: 400;" aria-level="1">Accept patient referrals from other physicians.
