<div><strong>Job Responsibilities</strong></div>
<ul>
<li>Delivers high-quality therapy services following the relevant Standards of Practice</li>
<li>Assessing the patient and creating a personalized treatment plan with short and long-term objectives</li>
<li>Monitoring and informing the attending physician about the patient’s progress, response to treatments, and any changes in their condition; adjusting the treatment plan as necessary</li>
<li>Educating the patient, caregiver, and other healthcare team members on techniques to enhance and correct disabilities</li>
<li>Maintaining regular communication with physicians and other healthcare team members</li>
<li>Developing achievable long-term goals for patient discharge that can be adjusted as needed</li>
<li>Preparing the patient and caregiver for post-discharge care</li>
<li>Informing the Clinical Director of any significant patient, family, or physician concerns</li>
</ul>
<div><strong>Requirements & Qualifications</strong></div>
<ul>
<li>Must possess a valid state license, CPR certification, driver’s license, and car insurance.</li>
<li>Comprehensive understanding of professional theory, practice, techniques, principles, and tools.</li>
<li>Strong verbal and written communication skills. Proficient in documenting patient care.</li>
<li>Able to establish and maintain relationships with patients, healthcare professionals, physicians, and the community.</li>
<li>Minimum of one year of experience is mandatory.</li>
