As a Social Worker in our Hospital Team, your key responsibilities will include:
Assessing patients' social, emotional, and environmental needs to develop comprehensive care plans.
Providing emotional support and counseling to patients and their families, ensuring they feel heard and understood.
Collaborating with medical staff to coordinate discharge planning and ensure a smooth transition to post-hospital care.
Connecting patients with community resources and support services, including financial assistance and rehabilitation programs.
Documenting assessments, interventions, and outcomes in compliance with hospital policies and regulatory requirements.
To be considered for this position, you must hold a recognized social work qualification (e.g., Degree in Social Work) and be registered with Social Work England. Essential skills include strong communication and interpersonal abilities, critical thinking, and a deep understanding of social welfare policies and healthcare systems. Experience in a hospital or healthcare environment is highly desirable.
