About the Role
Eden Brown Synergy is currently looking for an experienced & dedicated Advance Practitioner for Gateshead Council to join their Children with Disability Team.
Payrate: £34.45 p/h Umb
Duties and Responsibilities:
Undertaking Parenting Assessments
Undertaking Risk Assessments
Undertaking Educational Needs Assessments
Carrying out home visits for monitoring purposes/ to promote the welfare of and to safeguard the children
Liaising/ Working close with partnership agencies to support the families and meet the children's/ young people's needs
Convening and chairing Core Group Meetings
Producing Initial Child Protection Conference reports
