Pay: $27.16 per hour Must reside in nearby counties within (Cook, Dupage, Will, Lake, Winnebago) This is a hybrid position with remote work options. Case Management Coordinator Job details Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment. Utilize critical thinking and judgment to inform the case management process. Conduct comprehensive evaluations of referred members' needs and eligibility. Coordinate and implement care plan activities and monitor progress. Consult with case managers and medical directors to overcome barriers to care. Engage members to promote participation in their healthcare decision-making. Document care management processes in compliance with regulatory guidelines. Case Management Coordinator Requirements: Bachelor's degree in behavioral health or human services required (psychology, social work, nursing, etc.) or non-licensed master's level clinician.
