Eden Brown synergy are currently recruiting for Wiltshire Council who are looking for Occupational Therapist - Reablement Team Job Title: Occupational Therapist - Reablement Team Location: Wiltshire (Based at Monkton Park, Chippenham or County Hall, Trowbridge) Hospital Sites: Great Western Hospital (Swindon) & Royal United Hospital (Bath) Employer: Wiltshire Council Rate: £36.00 per hour Contract: Full Time Role Overview Wiltshire Council is seeking an experienced Occupational Therapist to join the Reablement Team, working in close partnership with acute hospital colleagues to facilitate timely and safe discharges into community-based reablement services. This is a fast-paced, high-demand environment requiring confident clinical decision-making and effective multidisciplinary collaboration. You will play a key role in supporting discharge planning pathways from acute settings into short-term reablement interventions. Key Responsibilities Work in partnership with hospital teams at Great Western Hospital (Swindon) and Royal United Hospital (Bath). Support discharge planning and facilitate safe transitions into the Reablement Service. Undertake functional assessments to determine reablement suitability and intervention requirements. Develop outcome-focused, strengths-based reablement plans. Collaborate with MDT professionals, discharge coordinators, and community teams. Manage a dynamic caseload within a busy acute discharge setting. Essential Requirements Qualified Occupational Therapist with current HCPC registration. Demonstrable experience working within hospital discharge, acute, or reablement services. Proven ability to operate effectively in a fast-paced and pressurised environment. Strong assessment, risk management, and decision-making skills. Ability to work across multiple sites as required.
