Job Description
  • Provide coordinated care to patients of all ages in a home care environment.
  • Plan, implement, and evaluate individualized patient care plans to help patients develop, regain, or maintain their ability to perform daily activities.
  • Evaluate occupational roles and performance for patients with physical, psycho-social, and cognitive impairments.
  • Teach patients skills and techniques for using adaptive equipment.
  • Document and complete a Plan of Care (POC) and maintain accurate clinical records.
  • Assess and address patient needs through direct therapeutic interventions.
  • Collaborate with the interdisciplinary team on patient status.
Critical Information
  • Flexible schedule with local travel required.
Education/Licenses Needed
  • Current state license as an Occupational Therapist - Required
  • Graduate of an accredited school of occupational therapy with a Bachelor of Science degree in Occupational Therapy.
  • Proof of current CPR certification - Required
  • Current driver's license - Required
  • Reliable transportation and insurance - Required
  • Two years experience as an Occupational Therapist in an acute care or rehabilitation setting - Required
