Home Health LPN Compensation • The pay for this position is $37.30 per hour + $0.54/mile mileage reimbursement • Benefits are available to full-time employees after 90 days of employment • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Home Health LPN Highlights • Location: Virginia Beach (Home Health – travel within assigned counties) • Counties Covered: Chesapeake, Virginia Beach, Portsmouth, Suffolk, and Norfolk • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs • The required availability for this position is Monday – Friday, 8:00 AM – 5:00 PM • On-call and overtime may be required occasionally Home Health LPN Responsibilities • Provide direct patient care following each client’s established plan of care while maintaining dignity and independence • Monitor vital signs and report abnormal findings to the appropriate medical staff • Administer medications, oxygen, feedings, and ostomy care as required • Perform wound care and assist with chronic disease management as needed • Complete routines established by other care professionals including PT, OT, and Speech Therapists • Maintain and enforce infection control procedures and report potential communicable diseases • Document all nursing care, patient progress, and communication accurately • Order and manage medical supplies, medications, and feedings as needed • Ensure all patient needs are met according to physician instructions • Monitor and document incidents involving patients and ensure reports are completed accurately and promptly • Ensure medical equipment is functioning properly and report faulty equipment for repair Home Health LPN Requirements • Active LPN license in the State of Virginia • Ability to work onsite and travel within the assigned service counties • High School Diploma or GED • Attendance is mandatory for the first 90 days Home Health LPN Preferred Qualifications • Experience with wound care or wound vacs • IV therapy experience • Experience performing lab draws • Experience with Homecare Homebase (HCHB) • Strong bedside manner and patient interaction skills If you think this Home Health LPN position is a good fit for you, please reach out to me – feel free to call, e-mail, or apply to this posting!
