Your recruiter will provide the most up-to-date details during the hiring process.<h3>How you’ll make an impact in this role </h3><ul class="gdAMzb"><li class="rnc2Gd">Champion Patient Success: Proactively implement, evaluate, and modify individualized patient care plans to achieve the best possible outcomes.</li><li class="rnc2Gd">Be the Patient Advocate: Continuously monitor, accurately document, and promptly communicate changes in patient status to the care team, acting as their voice and primary protector.</li><li class="rnc2Gd">Lead the Coordinated Effort: Serve as the central coordinator, connecting and facilitating seamless, well-orchestrated care across all medical disciplines to ensure a unified patient experience.</li><li class="rnc2Gd">Execute with Precision: Meticulously carry out physician and advanced practice provider orders, along with specialized nursing protocols, ensuring every detail of the treatment plan is followed.</li><li class="rnc2Gd">Prepare for Tomorrow: Holistically assess patient discharge needs and proactively collaborate with the multidisciplinary team to plan for a safe, successful transition home or to the next level of care.</li></ul><h3>What minimum requirements you’ll need </h3>Licensure / Certification / Registration:<ul><li>BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
