<p data-path-to-node="5">This case involves a resilient 1-year-old girl with a history of Trisomy 21 and several cardiac repairs (AVSD/PDA). She is currently preparing for discharge from the hospital, and we are building her "dream team" to ensure she has a safe and happy return home.</p>

<h3><strong>Responsibilities</strong></h3>
<ul>
<li>Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.)</li>
<li>Monitor and document vitals, symptoms, and changes in condition</li>
<li>Assist with ADLs such as repositioning, hygiene, mobility, etc.</li>
<li>Support patient well-being with compassionate and attentive care</li>
<li>Collaborate with families, caregivers, and the healthcare team<br><br></li>
</ul>
<h3><strong>Qualifications</strong></h3>
<ul>
<li>Active RN or LPN license in the state of Michigan</li>
<li>Experience in home health, private duty, or long-term care preferred</li>
<li>Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required)</li>
<li>Strong communication skills and a passion for patient-centered care<br><br></li>
