Home Health Physical Therapist PT - Full-time

We are seeking a dedicated and experienced Physical Therapist (PT) to join our dynamic team of healthcare professionals providing exceptional home health services in Ellis County. This full-time position offers the opportunity to work independently and make a significant impact on patients' lives by helping them regain their independence and improve their quality of life. Key Responsibilities: Conduct comprehensive patient assessments to develop personalized care plans. Provide therapeutic treatments and interventions in the patient's home environment. Collaborate with patients, families, and healthcare teams to achieve optimal outcomes. Document patient progress and modify treatment plans as necessary. Educate patients and their families on exercises and strategies to enhance mobility and function. Ensure compliance with all regulatory and professional standards of care. Qualifications: Valid state license as a Physical Therapist. Proven experience in a home health setting is preferred. Strong communication and interpersonal skills. Ability to work independently and manage a flexible schedule. Excellent organizational skills and attention to detail. CPR certification is required. Valid driver’s license and reliable transportation.
