Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for relevant medical history and precautions
• Establish measurable goals and develop treatment plans
• Reassess and modify treatment plans as necessary
• Communicate patient needs and progress to the treatment team and family
• Consult with medical providers regarding treatment and patient progress
• Recommend and secure adaptive equipment for patients
• Maintain and submit documentation according to regulations
• Adhere to professional practice standards and legal regulations
• Supervise Physical Therapist Assistants and Aides
• Participate in staff development programs and obtain continuing education

Requirements of the Physical Therapist

• MD PT license
• Home health experience
• Knowledge of HCHB & Oasis
