CAN Community Health is now hiring a Mobile HCV Community Health Worker I
Schedule: Full-Time | Day Shift | Monday-Thursday 8:00 am - 5:00 pm Friday 8:00 am - 12:00 pm
Salary: $21-$24 per hour based on experience

What You’ll Do
The Mobile Community Health Worker provides patient-centered HIV/STI/Hepatitis C testing and health education; supports clinical care on mobile and clinic-based units; delivers risk reduction counseling and PrEP/Hepatitis C linkage; and conducts community outreach to expand access to prevention and treatment. This role focuses on direct service delivery in outreach environments while ensuring that all documentation and practices comply with professional and legal standards.

Education/Professional:
