About the Role
Facilitates communication between the patient, their primary care physician and specialists to improve clinical outcomes. Works collaboratively with the patient to foster self-management and compliance to their clinical plan of care. Works collaboratively with physician, hospital and community resources to support the patient's clinical plan of care.<br/><br/>Education<br/>Bachelor's Degree in Nursing Required<br/>Master's Degree in Nursing Preferred<br/><br/>Experience<br/>3 years experience in disease or case management services with focus on telephonic<br/>management, medical reconciliation and ambulatory care coordination Required<br/>General Experience with clinical, hospital-based information systems Required<br/><br/>Licenses<br/>PA Registered Nurse License Required<br/>Multi State Compact RN License Required Or
