Alexis Wordlaw-Conley awordlaw-conley@alinestaffing.com  586-422-1748  RN Case Manager Position Details Location: 100% On-Site – Shiloh, IL 62269 Coverage Area: St. Clair, Madison, Clinton, Randolph, Bond, Monroe & part of Washington County Schedule: Monday–Friday | 8:00 AM – 5:00 PM On-Call: Monthly rotation Pay Rate: $56.14 per hour Mileage Reimbursement: $0.43 per mile Hours: 40+ per week (Full-Time) RN Case Manager – Home Health, you will: Perform comprehensive assessments of high-risk members Develop and implement individualized care plans in collaboration with primary care providers Evaluate cost-effective and appropriate levels of care Coordinate care transitions with physicians, social workers, discharge planners, and claims teams Arrange non-medical support services (housing, transportation, etc.) Engage specialty resources to improve patient outcomes Maintain detailed documentation of clinical, functional, and fiscal outcomes Promote health education and illness prevention Advocate for patients while ensuring privacy and confidentiality Intervene promptly to prevent adverse outcomes RN Case Manager Requirements Associate or Bachelor’s Degree in Nursing Active Multi-State RN License Case Management Certification OASIS & HCHB experience CPR/BLS Certification Current TB Test Wound Care Competency Valid insurance & ability to travel locally Strong clinical judgment and analytical skills Commitment to mandatory attendance during first 90 days
