Clinical Scope
- Coordinate patient transfers into, out of, and within the healthcare system
- Serve as primary liaison between referring facilities, receiving physicians, nursing leadership, and bed management
- Assess transfer requests and coordinate patient placement based on clinical needs and bed availability
- Facilitate communication between transferring and receiving teams to ensure continuity of care
- Arrange transportation and logistical needs for patient transfers
- Document transfer activities accurately and maintain detailed records for quality assurance and regulatory compliance
- Collaborate with multidisciplinary teams to promote efficient patient flow
Requirements
- Active Registered Nurse (RN) license in good standing
- Recent Epic EMR experience within the past 12 months
- Excellent communication, organizational, and critical thinking skills
- Ability to prioritize and manage multiple transfer requests effectively
- Knowledge of patient placement, bed management, and healthcare transfer regulations preferred
