About the Role
We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's SW - Review/Extra Care Team in Salford. This role is pivotal in ensuring that the needs of vulnerable adults are met through comprehensive assessments and effective care planning. As part of a dynamic team, you will contribute to the delivery of high-quality support services that empower individuals to live independently and with dignity.
Key responsibilities include:
Conducting thorough assessments of individuals’ needs and developing personalized care plans.
Reviewing and monitoring existing care packages to ensure they remain effective and appropriate.
Collaborating with multidisciplinary teams, including healthcare professionals and community organizations, to provide holistic support.
Advocating for the rights and wishes of service users, ensuring their voices are heard in all decisions.
Maintaining accurate and up-to-date records in accordance with legal and organizational standards.
To be successful in this role, candidates must possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
Registration with Social Work England.
Proven experience in adult social care, particularly in review and assessment processes.
Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
Excellent organizational skills and the ability to manage a caseload effectively.
