If you are interested in this RN Field Case Manager position, please contact Austin Faris at 586-710-7941 or afaris@alinestaffing.com RN Field Case Manager Compensation • The pay for this position is $56.15 per hour • Mileage reimbursement of $0.54 per mile • Benefits are available to full-time employees after 90 days of employment • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates RN Field Case Manager Highlights • This position is a contract assignment with potential to hire permanently based upon attendance, performance, and business needs • The required availability for this position is Monday – Friday, 8:00 AM – 5:00 PM • No on-call or overtime required • Field-based role covering Chesapeake, Virginia Beach, Portsmouth, Suffolk, and Norfolk RN Field Case Manager Responsibilities • Perform comprehensive clinical assessments of high-risk members • Collaborate with primary care providers to implement individualized care plans with targeted interventions • Identify and reassess appropriate, cost-efficient levels of care for members • Arrange non-medical support services such as housing and transportation to support treatment compliance • Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals • Engage specialty resources when needed to achieve optimal patient outcomes • Maintain detailed documentation of clinical, functional, and fiscal outcomes during the case management process • Identify opportunities for health promotion and illness prevention • Intervene quickly to minimize adverse patient outcomes when prevention is not possible • Act as a patient advocate while protecting privacy and confidentiality RN Field Case Manager Requirements • Associate’s or Bachelor’s Degree in Nursing (or related field) • Active, unrestricted Registered Nurse (RN) license in Virginia • Case Management certification required • Ability to analyze complex medical information and make rational clinical decisions • High School Diploma or GED • Attendance is mandatory for the first 90 days RN Field Case Manager Preferred Qualifications • OASIS assessments experience • Wound care and wound vac experience • IV therapy and lab draws • Chronic disease management experience If you think this RN Field Case Manager position is a good fit for you, please reach out to me – feel free to call, e-mail, or apply to this posting!
