Durham County - Sr. QSW - Connecting Families Team

Eden Brown Synergy is currently looking for an experienced & dedicated Sr. Social Worker for Durham County Council to join their Connecting Families Team.

Payrate: £32.73 p/h Umb

Duties and Responsibilities:

• Liaising/ Working close with partnership agencies to support the families and meet the children'/ young people's needs
• Preparing & implementing Care Plans
• Attending court and giving evidence when required
• Obtaining Interim Care Orders/Interim Supervision Orders
• Initiating Care Proceedings
• Completion of Care Proceedings
• Obtaining Special Guardianship Orders
• Producing Child Permanency Reports
• Reviewing Financial payments received by kinship carers/ or carers granted Residence Orders/Special Guardianship Orders
• Preparing looked after children review reports
• Initiating Personal Education Plans (PEP's)
• Producing Health Care Assessments and Health Plans
• Making referrals for Foster Placements
• Preparing children for placements
• Implementing Twin Tracking processes for children in care proceedings
