About the Role
We are seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Adult Social Care team in Liverpool. This position offers a competitive rate of £46.88 per hour and presents an exciting opportunity to make a significant impact on the lives of adults in our community. You will be responsible for assessing, planning, and implementing care plans that meet the diverse needs of service users while ensuring compliance with relevant legislation and best practices.
Your key responsibilities will include:
Conducting comprehensive assessments of service users’ needs and risks.
Developing and reviewing personalized care plans in collaboration with service users and their families.
Providing ongoing support and guidance to service users, ensuring they have access to appropriate resources and services.
Working closely with multidisciplinary teams, external agencies, and community organizations to facilitate effective service delivery.
Documenting case notes and maintaining accurate records in line with organizational policies.
To be successful in this role, you will need to meet the following essential qualifications and skills:
Hold a relevant social work qualification (e.g., Degree in Social Work, DipSW, CQSW).
Be registered with Social Work England.
Demonstrate substantial experience in adult social care, particularly in assessment and care planning.
Possess strong communication and interpersonal skills, with the ability to build rapport with service users and stakeholders.
Exhibit excellent organizational skills and the capacity to manage a caseload effectively.
