This role focuses on optimizing patient flow, reducing length of stay (LOS), and improving care transitions to enhance both patient outcomes and financial sustainability. font-size: 10pt;">Drive accountability for the implementation of Continuous Quality Improvement (CQI) methodologies by IP senior and site leaders to effectively reduce length of stay (LOS) while maintaining the highest standards of care.</span></li><li><span style="font-family: arial, helvetica, sans-serif; font-size: 10pt;">Hold IP senior and site leaders responsible for enhancing discharge planning processes to facilitate timely transitions and minimize avoidable delays.</span></li><li><span style="font-family: arial, helvetica, sans-serif; font-size: 10pt;">Ensure leadership participation in hospital-wide interdisciplinary rounds and escalation protocols to support efficient, data-driven decision-making.</span></li></ul><p style="margin: 0px;"><strong><span style="font-family: arial, helvetica, sans-serif; font-size: 10pt;">Transitions of Care & Post-Acute Coordination</span></strong></p><ul><li><span style="font-family: arial, helvetica, sans-serif; font-size: 10pt;">Hold senior leaders accountable for driving collaboration between hospital teams and post-acute care providers to ensure seamless patient transitions and continuity of care.</span></li><li><span style="font-family: arial, helvetica, sans-serif; font-size: 10pt;">Ensure senior leaders implement system-wide strategies to reduce readmissions and enhance care coordination across inpatient, outpatient, and home settings.</span></li><li><span style="font-family: arial, helvetica, sans-serif;
