<strong>Job Requirements </strong><br><p><strong style="color: rgb(0, 0, 0);"><u>General Summary</u></strong></p><p><span style="color: rgb(0, 0, 0);">Under supervision of an RN Case Manager, RN Utilization Manager or Social worker, the care management technician provides a variety of services to the department that requires thorough knowledge of departmental practices and procedures. The care management technician assists by supporting the core functions of the department and discharge planning.</span></p><br/><p><strong style="color: rgb(0, 0, 0);"><u>Principal Responsibilities and Tasks</u></strong></p><p><span style="color: rgb(0, 0, 0);">The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.</span></p><ol><li><span style="color: rgb(0, 0, 0);">Creates discharge packets for patients going to skilled nursing facilities.
