About the Role
Windsor & Maidenhead are seeking a dedicated and compassionate Social Worker to join our Hospital Discharge team. This role is vital in ensuring a smooth transition for patients leaving the hospital, providing essential support and resources to help them reintegrate into their homes and communities. You will work closely with healthcare professionals, patients, and their families to assess needs, create discharge plans, and facilitate access to necessary services.
Your key responsibilities will include:
Conducting comprehensive assessments of patients’ social and emotional needs.
Developing and implementing discharge plans in collaboration with multidisciplinary teams.
Coordinating with external agencies and community resources to ensure continuity of care.
Providing emotional support and guidance to patients and their families during the discharge process.
Maintaining accurate records and documentation in compliance with regulatory standards.
The ideal candidate will possess the following qualifications and skills:
A relevant degree in Social Work and a valid Social Work registration.
Proven experience in hospital discharge planning and working within a healthcare setting.
Strong assessment and analytical skills, with the ability to make informed decisions.
Excellent communication and interpersonal skills to effectively engage with diverse populations.
Empathy, resilience, and the ability to work under pressure in a fast-paced environment.
