About the Role
Job Description: Licensed Practical Nurse (LPN) responsible for providing patient care in a fast-paced outpatient setting, including rooming patients, triage, and clinical support under the direction of a provider. Ensures efficient patient flow while delivering high-quality, patient-centered care. Responsibilities: Room patients and maintain efficient patient flow for scheduled and walk-in visits Perform triage (in-person and phone) and prioritize patient needs per protocol Obtain and document vital signs, medical history, and clinical information in the EMR Communicate patient concerns and clinical findings to providers Assist providers with exams, procedures, and patient care Administer medications, immunizations, and treatments as ordered Process prescription refills and patient calls/messages Perform or assist with in-office testing and specimen collection Provide patient education and support for care plans and follow-up Maintain accurate, timely documentation and uphold confidentiality standards Qualifications: Active, unrestricted LPN license in Vermont Current CPR certification Strong clinical assessment and triage skills Excellent communication and interpersonal skills Ability to multitask and thrive in a fast-paced environment Core Competencies: Patient-focused and team-oriented Strong organizational and time-management skills Ability to prioritize and adapt quickly Professional, dependable, and detail-oriented Work Environment/Physical Requirements Ability to stand for extended periods and assist with patient movement Capable of responding to patient care needs quickly and efficiently Comfortable working in a busy clinical setting with multiple demands
