MEDICAL Social Worker

<p>We are an acute care medical center and a member of a large academic neighboring Health System looking for a Medical Social Worker to join our growing Care Management department. As the Medical Social Worker in the Care Management dept you will perform<br /> the initial comprehensive assessment on admission in accordance with the Care Management Department policy, screening all patients by utilizing established tools for high risk indicators to ensure high risk patient populations receive the appropriate supportive services for discharge to prevent readmission and assess all populations for potential discharge planning needs. </p> <p>Position: MEDICAL Social Worker Care Management<br /> Location: White Plains,, NY<br /> Hours: Mon &#8211; Fri 9-5 or 8a-4p; 1 weekend (Sat/Sun) per month, alt<br /> Salary: $87,000 &#8211; $130,000 Sign On Bonus : $5,000</p> <p>Responsibilities:</p> <p>Completes a comprehensive initial assessment and performs all activities for multidisciplinary care coordnation at the intra-hospital level of care and inter-hospital level of care.<br /> Performs all transitional planning activities including but not limited to; Patient/caregiver education, Arrangement of all aftercare, Commercial payer involvement and High utilizer patient root cause<br /> Communicates and collaborates all relevant patient information to appropriate health team members and ensures department goals of LOS reduction, readmission prevention, denial reduction, and satisfaction improvement are realized.<br /> Documents per department requirement and participates in Performance Improvement activities as needed.</p> <p>Requirements:</p> <p>Master of Social Work Degree from a School of Social Work accredited by the Council of Social Work Education.<br /> Licensed Social Worker (LMSW/LCSW) approved by the State Education Department of the University of the State of New York.<br /> Two years related experience in an acute care hospital setting<br /> NYS SCREEN Certification (obtained within initial 3 months of employment</p>
