About the Role
In conjunction with the Heart Failure team assumes responsibility for assessment, coordination, and education of the Heart Failure patient, on the inpatient and outpatient settings. Assists with delivering high quality, efficient medical care to patients in the Heart Failure Clinical Excellence Program within the Temple Heart and Vascular Institute. Works with complex heart failure patients and supports both the physicians and advance practitioners with patient care management. This includes assessments, interventions, referrals, documentation, telephone triage and pre-cert registration processes. Provides ongoing health education and counseling activities; prompt abnormal testing reporting and follow-up; and maintaining follow-up logs for record keeping purposes. May require travel to other TUH Campuses as needed. This position requires the successful completion of the CHFN or CHFN-K - Heart Failure Nursing Certification, within 2 years of date of hire.<br/><br/>Education<br/>Bachelor's Degree in Nursing from an accredited school of nursing. Required<br/>Master's Degree in Nursing from an accredited School of Nursing. Preferred<br/><br/>Experience<br/>1 year experience with a Heart Failure program Required<br/>3 years experience in cardiology nursing Required<br/>1 year experience in critical care nursing. Preferred<br/><br/>Licenses<br/>PA Registered Nurse License Required<br/>Cert Heart Failure Nurse Required or<br/>Cert Heart Failure Nurse KB Required<br/>Multi State Compact RN License Required Or
