– Assess the unique needs of adults with learning disabilities and autism, crafting tailored, person-centred care plans to enhance their quality of life. – Provide direct and empowering support with a focus on developing essential life skills, managing risks, and promoting independence. – Collaborate with multi-agency teams to ensure coordinated services and continuous monitoring of care plans. – Vigilantly identify and address safeguarding issues, managing risks to prioritize the safety and wellbeing of service users. – Facilitate smooth life transitions such as moving from childhood to adulthood or adapting to new living environments. – Maintain precise and comprehensive records, creating detailed reports on assessments, care plans, and individual progress. – Advocate vigorously for individuals’ needs and rights, liaising with families, carers, and external agencies. – Remain actively updated on legislation and best practices, participating in training to continually enhance your professional knowledge.
