Job Title: LPN/LVN - Rehabilitation - Travel

Location: Utica, NY

Pay: $1678.40 weekly

Shifts: 6:30am-2:30pm

Seven Healthcare are seeking an experienced LPN/LVN specializing in rehabilitation for a travel assignment in a long-term care (LTC) facility in Utica, NY. This role involves providing high-quality nursing care and support to residents with a variety of needs including MS, Orthopedic, Dementia, and Memory Care. The position requires flexibility with shifts and the ability to float within the facility.

Key Responsibilities of the LPN/LVN - Rehabilitation

• Provide direct nursing care to residents
• Administer medications and treatments as prescribed
• Monitor and report changes in residents’ conditions
• Collaborate with the healthcare team to develop and implement care plans
• Maintain accurate and detailed medical records

Requirements of the LPN/LVN - Rehabilitation

• Current LPN NY License & all active licenses
• 1+ years of experience in a LTC facility
• Up-to-date BLS certification
• Experience with EMR systems, specifically PointClick Care
• Valid driver's license
• Skills checklist matching specialty
• NURSYS report license verification
