About the Role
Position: Registered Nurse (RN)-{171193} Location: Albemarle, NC 28001 (covers Stanly County + parts of Davidson & Montgomery counties) Pay Rate: $42.62/hour Schedule: Monday-Friday, 8:00am-5:00pm Mileage Reimbursement: $0.41/mile Productivity expectation: 5-7 visits/day and/or 6 points/day Position Overview We are hiring a Registered Nurse (RN) for a Home Health-focused role supporting patients in the community. This RN will complete visits, assess patient needs, implement care plans, coordinate with providers, and ensure accurate clinical documentation. Key Responsibilities Perform patient assessments and implement individualized care plans Provide case management and coordinate care with multidisciplinary teams Monitor and manage medication processes (ordering, review, documentation, storage/access, disposal per policy) Communicate clinical updates to physicians, especially with condition changes Complete required daily/monthly/quarterly/annual nursing documentation Support infection control by maintaining equipment/supplies per guidelines Ensure appointments and follow-ups are scheduled and completed promptly Communicate with inpatient/discharge teams when patients are hospitalized Provide training for new employees and annual recertifications
