We are seeking an experienced Advanced Practitioner to join our Continuing Healthcare (CHC) team in the Wirral area. This hybrid position offers a competitive rate of £38.38 per hour and provides a unique opportunity to make a significant impact in the lives of individuals requiring ongoing health and social care support. You will play a crucial role in assessing, planning, and implementing care packages that meet the needs of clients while ensuring compliance with relevant legislation and best practice guidelines.

Your key responsibilities will include:

• Conducting comprehensive assessments to determine eligibility for CHC funding.
• Developing and reviewing person-centered care plans in collaboration with clients and their families.
• Providing expert advice and guidance to team members and stakeholders.
• Ensuring that all documentation and records are maintained accurately and in accordance with legal requirements.
• Participating in multi-disciplinary team meetings to promote effective communication and collaboration.

The ideal candidate will possess the following qualifications and skills:

• A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
• Current registration with Social Work England.
• Significant experience in Continuing Healthcare assessments and care planning.
• Strong analytical and decision-making skills, with the ability to work independently.
• Excellent communication and interpersonal skills to build rapport with clients and professionals.
