<strong>Outreach and Sustainment Officer (Housing)</strong><br><strong>Location:</strong> East London<br><strong>Contract:</strong> 3-Month Ongoing Contract<br><strong>Hours:</strong> 36 Hours per Week (Flexible Working Pattern)<br><strong>Rate:</strong> <strong>£24.12 per hour Umbrella<br></strong><br>About the RoleWe are currently recruiting for an experienced <strong>Outreach and Sustainment Officer</strong> to join a dedicated Housing team in East London.<br>This is an excellent opportunity to make a real difference by supporting people who are sleeping rough or at risk of homelessness. You will provide proactive, trauma-informed outreach, helping vulnerable individuals access accommodation, essential services, and ongoing support to enable them to achieve and sustain independent living.<br><br><strong>Key Responsibilities</strong><ul><li>Deliver proactive outreach to identify and engage people sleeping rough or at risk of rough sleeping.</li><li>Carry out assessments of individuals' housing, support, and welfare needs.</li><li>Provide practical advice, guidance, and signposting to housing, health, welfare, and specialist support services.</li><li>Support individuals moving into temporary accommodation, supported housing, or independent living, helping them to settle and sustain their tenancy.</li><li>Develop person-centred support plans focused on improving long-term outcomes.</li><li>Work collaboratively with internal teams and external partner agencies to coordinate effective support.</li><li>Conduct welfare and safety checks, ensuring risks are identified and managed appropriately.</li><li>Maintain accurate case records and ensure all work complies with relevant policies and procedures.</li></ul><strong>About YouTo be successful in this role, you will have:</strong><br><ul><li>Experience working with people experiencing homelessness, rough sleeping, or complex needs.</li><li>Knowledge of housing legislation, homelessness services, and safeguarding practices.</li><li>Experience carrying out support needs and risk assessments.</li><li>A trauma-informed, person-centred approach to supporting vulnerable individuals.</li><li>Excellent communication and relationship-building skills.</li><li>The ability to work independently while managing a varied caseload.</li><li>Strong organisational skills and experience maintaining accurate case records.</li></ul><br>If you're passionate about supporting vulnerable people and have experience within housing, homelessness, or outreach services, we'd love to hear from you.
